Nationwide, GEMS is seeking hungry independent sales reps who know how to close a physician customer. The sale is easy, commissions are uncapped and substantial, and your territory is anywhere you live or travel.
The work offered is commission only, with no base salary. The sales rep can work one hour a month – or ten hours a day. The commissions offered to the successful candidates are 35% of net income, with no ceiling on the potential amount paid the sales rep each month.
The Company:
Gibraltar Electro Medical Services manufactures and distributes the GEMS-TENS, a Transcutaneous Electrical Nerve Stimulator intended for the treatment of patients in chronic intractable pain. GEMS is a privately held company that has been manufacturing and distributing the GEMS-TENS for over 15 years.
The Product:
The GEMS-TENS is FDA approved for distribution in the USA. The GEMS-TENS has an assigned HCPCS code, and is not considered “experimental or investigational.” The GEMS-TENS comes complete with sufficient TENS supplies to last the average patient 7 – 10 days. We offer these devices to physicians nationwide for $29.00 when the physician purchases 2 or more. Reimbursement to the physician for a GEMS-TENS, when provided to a patient with individual or group health insurance coverage, is generally anywhere from the low $200s to the high $500s.
Target Physicians:
Orthopedists, pain management, podiatrists, osteopaths, neurologists, PM&R physicians, and anesthesiologists, are all familiar with our product that provides ‘Drug Free Pain Relief’ to the patient suffering from chronic intractable musculoskeletal pain.
Call Points:
Suggested points of sale are the physician’s office and/or the clinic specializing in pain management and functional improvement.
Territory and Quotas:
There is no assigned territory for any sales rep. The sales rep can sell anywhere he or she lives, travels, or works. Likewise, there is no quota for the sales rep to fill on a monthly basis. Keep working your W-2 job -- while getting paid substantial commissions on a 1099 basis.
The Sale and the Close:
This is a “buy and bill – prescribe and provide” product for the physician. The sales rep will sell the target physicians our GEMS-TENS for $29.00 when 2 or more are ordered. The sales rep will instruct the target physician which patients are suitable candidates to be provided a GEMS-TENS by diagnosis code(s) and type of insurance. The sales rep will instruct the physician how to correctly code and bill for provision of a GEMS-TENS. The sales rep will instruct the physician on how to refer a patient to GEMS for provision of the TENS supplies necessary to follow the prescribed treatment regimen.
Commissions Offered:
There is no commission paid on the sale of the GEMS-TENS. The sales rep is only paid commissions on the amount reimbursed for TENS supplies by the insurer. The amount of actual commissions paid the sales rep each month is 35% of the net income paid by the insurer the previous month. Gross income is defined as the amount reimbursed by the insurer. The hard cost of the TENS supplies, including delivery to the patient’s home, is $35.00 for a 3-month provision. The U&C charge by GEMS, for a 3-month provision of TENS supplies, is $439.00. Regardless of the amount reimbursed by the insurer, the hard cost of $35.00 is retained by GEMS off-the-top, and the remainder is considered to be net income. The sales rep is paid 35% of the net income.
Distinction from Competitors:
Our competitors generally fall into 2 distinct categories. One type of competitor is the ‘consignment competitor’ and the other type of competitor is the ‘catalog competitor.’
Examples of a ‘consignment competitor’ would be RS Medical or EMPI. This type of competitor leaves product with the physician’s office on a consignment basis. When the physician working with the ‘consignment competitor’ provides product to his/her patient, the physician refers the patient to the ‘consignment competitor’ who bills for the unit and the supplies. The physician realizes no income from working with a ‘consignment competitor.'
Examples of a ‘catalog competitor’ are internet medical supply companies. The ‘catalog competitor’ only sells product to the physician, without educating the physician on how to select which patients are good candidates, and without educating the physician on how to correctly code and bill for the unit. The physician usually pays more for the unit sold by the ‘catalog competitor’ and the physician is usually not paid by the insurer. This is because the physician is not instructed by the ‘catalog competitor’ on how to properly select patients where the insurer will determine the unit is “medically necessary,” and the physician is not taught how to properly code and bill for the unit and ancillary procedures.
A Distinction with a Difference:
The incentive for the physician to “buy and bill – prescribe and provide” is built in, as the physician will pocket all profits paid by the insurer over and above the low cost of the GEMS-TENS, $29.00. In today’s economic climate, the wise physician will do everything legally possible to maximize his/her income on each patient treated. This program is not for the physician primarily treating the Medicare or Medicaid beneficiary.
Once the GEMS-TENS is provided to a patient, that patient will require ongoing provision of TENS supplies. Reimbursement for the TENS supplies often continues for many years – resulting in continuing “passive commissions” to the sales rep.
Company Support:
GEMS will provide the sales rep with a ‘show and tell’ GEMS-TENS; extra disposable TENS supplies; promotional and marketing material; educational material delineating the diagnosis codes that provide a “medically necessary” foundation for provision of a GEMS-TENS; the proper HCPCS and CPT codes the physician should use to bill for the GEMS-TENS and ancillary procedures; business cards; and most importantly, telephonic training as needed.
The Successful Candidate:
The prospective sales rep will know the difference between an ICD-9 code, a CPT code, and a HCPCS code. Most importantly, the successful candidates will understand the concept of “medically necessary” is established by the diagnosis codes used to describe the patient’s injury or illness.
The Most Successful Sales Reps:
Our most successful sales reps have been those that already have a working relationship with many of our target physicians. These sales reps have been proficient in closing the physician in our “buy and bill – prescribe and provide” sales model.
Established or new medical sales representatives, who have an existing business relationship with our target physicians, are welcome to apply. <u>Basic knowledge of coding is essential --- but not required. Understanding how an ICD-9 code provides a "medically necessary" foundation - which is necessary for all insurers to issue reimbursement - distinguishes the successful candidate from the herd.
The sales rep who is a 'self-starter' and a 'go-getter' will succeed in this business.
The sales rep who already has an existing business relationship with numerous orthopedists, pain management, podiatrists, osteopaths, neurologists, PM&R physicians, and/or anesthesiologists - which physicians are already primarily treating the individual/group health insured - can easily earn in excess of $250k the first year.
To successfully “close” a physician, the proficient sales rep will conduct an in-service with the physician and teach that physician how to select patients to be provided a GEMS-TENS by diagnosis code and type of insurer; the proficient sales rep will teach the physician how to properly code and bill for a GEMS-TENS; and last but not least, the proficient sales rep will be successful in having that physician refer their patients to GEMS for provision of TENS supplies.
